What is Population Health

Population Health is an Affinity Health Group (AHG) program that takes a well-rounded approach to address the major factors that determine good health.

Our statewide team of highly qualified nurse practitioners and physician assistants works to prevent disease, educate patients and their families, promote better health and improve patient outcomes.

Medicare Advantage members through Blue Cross and Blue Shield of Louisiana have access to AHG Population Health as part of your health benefit. AHG partners with Louisiana Blue to perform comprehensive health evaluations in the comfort of your home or other location of your choice.

How Population Health Works

We offer flexible scheduling options, with visits in your home, on video via telehealth, or in person at our Monroe AHG Population Health Clinic.

During your visit, our nurse practitioner or physician assistant will:

  • Review your medical history and your medications
  • Get an overview of any symptoms or health concerns you may have
  • Conduct a standard physical exam
  • Perform or order preventive screenings as needed
  • Refer you to helpful Louisiana Blue and community resources as needed
  • Provide you with personalized health education and disease prevention strategies
  • Provide information on your health plan benefit benefits

Your population health visit is a benefit of your health plan and is provided at no extra cost.

AHG will send you and your primary care provider a summary of the visit and any test/screening results shortly afterward.

How Can AHG Population Health Benefit You?

Our experienced medical professionals at AHG Population Health offer resources to help you improve and maintain your health.

Population Health does not replace your primary care provider. We encourage regular visits with your primary care provider and can help you make an appointment. After your Population Health visit, we send your primary care provider a summary of the visit, as well as any test results.
