Affinity Health Group providers automatically file claims for all insurance companies. Because insurance coverage can vary from plan to plan, please understand that you are ultimately responsible for settling your account. Affinity Health Group will file secondary insurance. After processing by the insurance company, any patient responsibility will be billed to the guarantor.
This is not an exhaustive list of the insurance plans accepted by Affinity Health Group. Always check with your insurance company to be sure your providers are in-network.
We ask patients without insurance to pay in full at the time of service. We accept payments by cash, VISA or MasterCard.
